Each year the Office of Fraternity and Sorority Life will host an awards ceremony to celebrate and recognize the accomplishments of our community and members. The link to the awards listed below will be available when the award period opens.

Bid/Invitation Acceptance Agreement
This form is to be filled out by potential new members (PNMs) who hace accepted bid (or invitation to membership) from an organization and intend to start their new member joining process.

Reporting Civic Engagement Efforts
These forms are for reporting organizations efforts towards civic engagement. These efforts are reported on the semester reports at the end of each fall and spring semester.

Order of Omega Leadership Honorary Society
The Eta Mu chapter of Order of Omega was established in 1986 at the University of Utah as a honorary leadership society for fraternity men and sorority women of upperclassman status who have achieveed a 3.5 cumulative GPA or higher. The application link will be active once the society is accepting new members.
